30 million people on weight loss drugs. Almost none of them use a dedicated app. Zero creator marketing in the category (atm). This is the biggest untapped vertical in mobile.

30 million people worldwide are currently on GLP-1 drugs. Ozempic, Wegovy, Mounjaro, Zepbound. One in eight American adults is taking one right now.
The GLP-1 drug market is worth over $50 billion and growing at 20%+ per year. By 2030 it will be the largest pharmaceutical category in history.

And yet almost nobody in the app growth world is paying attention to the software layer being built on top of it.
Theres a GLP-1 tracking app called Shotsy doing $300K/month in revenue. 40K downloads per month. Nearly 1 million total users. And its ranked #152 in Health & Fitness on the US App Store.
Youve probably never heard of it.
Heres why this matters: the GLP-1 app category is where fitness apps were in 2018. Early, growing fast, massive TAM, and almost zero sophistication in how these apps acquire and retain users.
Nobody in this space is running creator campaigns. Nobody is doing UGC at scale. The entire category is growing on organic word-of-mouth from Reddit forums and doctor referrals.
If you're an app founder looking for your next vertical, or a growth operator looking for an underserved market, this is it.
The numbers.
Lets start with the drug market because the app opportunity is a direct function of it.
· 30 million people currently on GLP-1s globally
· 1 in 8 US adults currently taking a GLP-1 drug
· 27% of global adults are eligible for GLP-1 treatment
· $50-80 billion drug market in 2026, growing 20%+ Compound Annual Growth Rate
· 55 million Americans projected on GLP-1s by 2035
Now the app side:
· Shotsy: $300K/month revenue, 40K downloads/month, ~1M total users
· MeAgain: 250K+ users, 4.81 stars, subscription model
· Pep: 11K+ downloads, growing fast
· PeptideCalc: 4.99 one-time purchase, 4.9 stars, niche but loyal
· Total category revenue: estimated $5-6M/year across all apps
That last number is the one that should jump out. $5-6M/year serving a market of 30 million users means less than 1% penetration. The top app in the space is making $3.6M/year in a $50B+ market.
For comparison: Cal AI hit $50M ARR serving a calorie tracking market that's a fraction of the size. The GLP-1 app opportunity is bigger and less competitive.

Why nobody is marketing these apps.
I looked at every major GLP-1 app and checked their growth channels. Here's what I found:
· Zero active UGC or creator campaigns on TikTok, Reels, or Shorts
· No visible influencer partnerships
· No paid social ad libraries on Meta or TikTok
· Growth is almost entirely organic: Reddit (r/Mounjaro, r/tirzepatidecompound), doctor referrals, and App Store search
This is exactly where fitness apps were 5 years ago. Before Cal AI spent $0 on ads and got 250M views. Before apps like Hevy and Lose It built creator networks. Before UGC became the default growth engine for health apps.
The GLP-1 app space hasnt had its UGC moment yet. When it does, the first mover will dominate.
Why creator content is perfect for this vertical.
GLP-1 content is already exploding on social. There are millions of organic posts about Ozempic, Mounjaro, weight loss journeys, side effects, meal planning on GLP-1s. The audience exists. The content demand exists. Nobody is funneling it to an app.
Here's why creator marketing works especially well here:
## 1. The before-after format is built into the product.
Weight loss is the most visual transformation category that exists. Every GLP-1 user has a before-after story. Creators showing their 3-month or 6-month transformation while crediting the app that helped them track it is the most natural ad format possible.

Cal AI understood this. Their best-performing content was always before-after. GLP-1 apps havent figured it out yet.
## 2. The audience is desperate for guidance and community.
Starting a GLP-1 is confusing and scary. Dosing schedules, reconstitution (for compounded), injection sites, side effects, protein requirements, hydration. People are overwhelmed.
A creator who says "heres the app I use to track everything" provides immediate value. Its not a pitch, its a lifeline. That conversion intent is stronger than almost any other vertical.
## 3. The stigma creates intimacy.
Theres still stigma around weight loss drugs. People don't always talk about it openly. But they DO engage with creator content from people who share their experience authentically.
Creators who are open about their GLP-1 journey build unusually deep trust with their audience. That trust transfers directly to the app they recommend. Conversion rates in stigmatized health verticals are typically 2-3x higher than general health content.
## 4. Medical content has higher retention.
GLP-1 tracking is a daily habit tied to a medical routine. Retention curves for medication tracking apps are significantly flatter than general fitness apps.
Higher retention = higher LTV = more budget available for acquisition. The unit economics in this vertical are better than most founders realize.
## 5. Compounded peptides create a power-user segment.
Beyond branded GLP-1s (Ozempic, Wegovy), theres a massive compounded peptide market. These users need reconstitution calculators, dosing protocols, and inventory tracking.
This is a power-user segment willing to pay $5-15/month for premium features. PeptideCalc already charges $4.99 one-time and has 4.9 stars. A subscription model with creator-driven acquisition could scale this significantly.
The opportunity for founders.
If I were building a GLP-1 app from scratch in 2026, heres the playbook:
· Start with the compounded peptide niche (highest willingness to pay, underserved)
· Build the best reconstitution calculator + tracking combo
· Launch with 5-10 creators on TikTok posting GLP-1 journey content
· Use the before-after format as the core creative
· Target r/Mounjaro, r/tirzepatidecompound with organic posts
· Price at $4.99/month or $29.99/year (the market supports it)
· Expand to branded GLP-1 users once the creator engine is running
Cal AI went from a tracking app to $50M ARR by adding a creator engine. The GLP-1 category is the same product type, same user behavior, same before-after content format, with a market thats 10x bigger and 5 years behind on marketing sophistication.
## The opportunity for growth agencies.
This is also a massive opportunity for agencies that specialize in creator-driven app growth (full disclosure: thats us).
The GLP-1 vertical has:
· High LTV users (medical adherence = daily opens)
· Natural content formats (before-after, day-in-my-life, tutorial)

The first agency to build a GLP-1 creator network will have a significant head start (we already started ; ) ). The creators who do this content well dont exist yet at scale. Building that network now is the moat.

The bigger picture.
Every major health trend creates an app category that follows a predictable arc:
· Phase 1: The drug/treatment goes mainstream (GLP-1s: 2023-2025)
· Phase 2: Tracking apps appear, grow on organic/Reddit (where we are now)
· Phase 3: One app adds a creator engine and separates from the pack
· Phase 4: Category consolidation, acquisitions, $50M+ outcomes
We saw this exact arc with calorie tracking (Cal AI), meditation (Calm/Headspace), and cycle tracking (Flo). Each time, the app that won Phase 3 owned the category for years.
GLP-1 apps are at the end of Phase 2. Phase 3 hasn't started yet. The window is open.
30 million users. $50B+ drug market. Less than 1% app penetration. Zero creator marketing.
